The Pakistan Demographic and Health The Pakistan Demographic and Health Survey is a stratified, two-stage random sample covering urban and rural areas of the four provinces of Pakistan and Gilgit Baltistan, defined as such in the 1998 Population Census. The survey included 500 primary sampling units, covering 12,943 out of 13,944 sampled households, 13,558 out of 14,569 ever-married women and 3,134 out of 3,991 ever-married men.
Four types of questionnaires were used in the PDHS: The Community, Household, the Woman's and Husband’s Questionnaire. In addition, interviewing teams measured the height, weight and arm circumference of all respondents' children under age five. (for details please see Sample Design Section of Chapter 1, Main Report).
